Position Summary

Qualifications Required

  • One year experience in graphic design or related field

  • Advanced proficiency in Adobe Creative Suite (Adobe Photoshop, Illustrator, InDesign), Web Content Management System (CMS) Drupal or similar CMS Platform

Position Summary

Position Summary

The Wellness Care Navigator (WCN) position provides an exciting opportunity to support the development and integration of a Client Centered Nutrition Home model within the Project Open Hand (POH) Wellness program. The WCN will integrate lessons learned from an ongoing Administration for Community Living (ACL) demonstration project to assist POH Wellness Program clients in addressing social determinants of health that impact their ability to eat a nutritious diet to manage their chronic condition(s). 

The WCN will be responsible for conducting wellness assessments, assisting clients in establishing SMART goals related to their chronic condition, referring clients to external and internal resources, assessing client adherence and engagement, conducting outreach to clients who are not meeting adherence goals, and working one-on-one with clients to improve their program adherence and health outcomes. This role will work closely with other members of the Client and Nutrition Services teams, and will collaborate with internal departments and external partners to reduce barriers to clients accessing POH services. The WCN will conduct outreach and build partnerships with health care providers in San Francisco to improve the client referral and recertification processes, working to identify and remove barriers for referring providers and clients. The WCN will develop partnerships with social service providers in San Francisco and facilitate warm hand-off referrals of POH clients to external services. 

This position is ideal for a highly organized and compassionate professional with the ability and skills to engage clients as well as external and internal partners. 

Qualifications Required

  • Bachelor’s Degree in a Human Services related field or commensurate experience.
  • Three years of experience providing direct services in a Social Service environment, specifically with older adults and/or vulnerable populations with multiple diagnoses (i.e., substance abuse, mental illness, critical illness).

Position Summary

The Manager, Events will be the primary lead on all internal and external events that benefit Project Open Hand.  Key responsibilities include all aspects of managing events including logistics, recruiting, training, and coordinating event volunteers, implementing event marketing and communications strategies, cultivating relationships with and soliciting event sponsors, in-kind support, and donors, coordinating event contractors and vendors, and record keeping.  The management of events may include working with other Development and Project Open Hand staff to coordinate their functions and contributions to a successful event.

Qualifications Required

  • 3-5 years of event management experience preferably in San Francisco Bay Area
  • 3-5 years of nonprofit experience, including but not limited to fundraising, donor management, volunteer recruitment, and event creation and oversight

The Finance team is currently offering an Accounting Internship for a driven individual that is looking for hands-on accounting experience.  Interested applicants must be able to work at least 4 hours per week.   • Processing Donations: work with members of our Development team to open, process and reconcile gifts received by mail.

  • Other administrative duties as assigned.
