Community Nutrition Program

San Francisco resident seniors (60 years or older) and adults with disabilities (18 – 59 years old) are invited to join us for lunch at community sites located across the city of San Francisco, CA. Community Nutrition Sites

CalAIM Community Supports Programs

Medi-Cal members of Anthem Blue Cross and Alameda Alliance for Health may be eligible for our services through Community Supports if they have a chronic condition(s) such as but not limited to diabetes, cardiovascular disorders, congestive heart failure, stroke, chronic lung disorders, human immunodeficiency virus (HIV), cancer, gestational diabetes or other high-risk perinatal conditions, and chronic or disabling mental/behavioral health disorders. Wellness Programs

Project Open Hand Wellness Programs are available to residents of San Francisco and Alameda County who are eligible based on a primary diagnosis. If you do not have a Wellness Program eligible diagnosis, you may be eligible for our Community Nutrition Program or Project Age Well. Eligible diagnoses (effective 03/15/22) include: 

San Francisco County

  • Cancer (any stage)
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Congestive Heart Failure (CHF)
  • Coronary Artery Disease (CAD)
  • Type 1 or Type 2 Diabetes, with an HbA1c of 8.0% or above
  • Hepatitis C
  • HIV/AIDS
  • Recent major surgery, within 30 days of discharge (6-week service)

Alameda County

  • HIV/AIDS
  • Hepatitis C
  • Recent major surgery, within 30 days of discharge (6-week service)

If you are a member of San Francisco Health Plan and have one of the below diagnoses, you may also be eligible for services. 

  • Pre-diabetes (age 14+) 

  • Diabetes (age 14+) 

  • End-Stage Renal Disease (age 18+) 

  • COVID-19 affected (long-haul Covid or new diagnosis, age 18+) 

  • Complex medical conditions needing nutritional support (age 18+) 

  • Acute hospital discharge (age 18+) 

    • Malnutrition, diagnosed during hospital stay 

    • Nutrition support needed post discharge 

    • Food insecurity 

    • Assessment for high risk of re-admission 

How to Apply

Step 1: Client must have an eligible diagnosis.

Step 2: A licensed medical provider must fill out and sign the application. This can be: RN, NP, MD, DO, RD or RDN, LCSW or PA. Client’s verbal consent on the application is OK.

Step 3: Client or provider sends application to Project Open Hand via fax, email, mail or in person. All relevant information is on the application.

Step 4: Project Open Hand's Client Services will evaluate eligibility and contact the client to set up services if eligible. If the client is not eligible, Client Services will contact the referring party.
